Pre-Visit Questionnaire
Pet Care Built Around the Root Cause
This questionnaire is a KEY Component in helping us identify your pet’s underlying imbalances and past events that could be contributing to current health concerns.
Returning client? Skip the form and email Dr. Parker to request your next vet care appointment.
Step One: Share Your Pet’s Story
Every new client begins with a detailed questionnaire. This helps Dr. Parker to understand your pet’s full history, environment, and current concerns before scheduling a veterinary consultation. Taking time here ensures your vet care appointment is thoughtful and tailored to your pet’s needs.
Step Two: Review & Scheduling
After reviewing your responses, Dr. Parker will personally follow up via email to discuss next steps. This may include scheduling a house call or phone consultation, or recommending alternative vet care if specialized intervention is needed.
Step Three: Comprehensive Consultation & Guidance
During your veterinary consultation, Dr. Parker will evaluate the whole picture and provide personalized guidance. Afterward, you’ll have a clear path forward rather than a list of unexplained recommendations.
This form is easiest to complete on a computer. After submitting, please make sure you see the confirmation page.

Frequently Asked Questions
Understanding Our Approach to Pet Care
What is functional veterinary care?
We focus on the function and functionality of your pet. We use our holistic approach to find what works best for your pet and use whatever tools are necessary to help them feel better and become healthier.
Is the exam just as thorough as a visit to the regular vet clinic?
We do house calls so that we can be more thorough! That way, we can spend more time with you and your pet in their home environment, where we may find clues about why they are sick. We focus on you and your pet during the exam, rather than being distracted by other appointments and patients.
Do you offer telemedicine, online consultations or phone consults?
We provide phone consultations for clients outside of our house-call area. We can often help you find what is missing by thoroughly reviewing and discussing your pet’s medical & life history, test results, and prior treatments, and then recommending different options via a phone consultation.
Do you do routine/annual/wellness exams?
Do you provide vaccinations?
No. Instead, we take a holistic-minded approach to health & wellness & prevention.
I am specifically seeking acupuncture (or chiropractic) for my pet - will you do that?
Yes. We use a thorough questionnaire, a detailed discussion with you, and a comprehensive exam of every patient to determine the underlying core problem. Then we choose a treatment modality based on our findings. Sometimes it’s acupuncture, chiropractic, laser therapy, or rehab exercises. Other times, it’s all about herbs and nutrition. However, it’s usually some combination of these.
