Pre-Visit Questionnaire

Pet Care Built Around the Root Cause

This questionnaire is a KEY Component in helping us identify your pet’s underlying imbalances and past events that could be contributing to current health concerns.

Returning client? Skip the form and email Dr. Matt to request your next appointment.

Every new client begins with a detailed questionnaire. This helps Dr. Matt to understand your pet’s full history, environment, and current concerns before scheduling a consultation. Taking time here ensures your appointment is thoughtful and tailored to your pet’s needs.

After reviewing your responses, Dr. Matt will personally follow up via email to discuss next steps. This may include scheduling a house call or phone consultation, or recommending alternative care if specialized intervention is needed.

During your consultation, Dr. Matt will evaluate the whole picture and provide personalized guidance. Afterward, you’ll have a clear path forward rather than a list of unexplained recommendations.

This form is easiest to complete on a computer. After submitting, please make sure you see the confirmation page.

The following Traditional Chinese Medicine questions help us determine your pet’s “personality” and what problems they may be predisposed to.

Frequently Asked Questions

Understanding Our Approach to Pet Care

We focus on the function and functionality of your pet. We use our Holistic approach to find what works best for your pet and use whatever tools necessary to help them feel better and become healthier.

We do house calls so that we can be more thorough! We get to spend more time with you and your pet in their home environment, where we may find clues as to why they are sick. We focus on your pet and you during the exam instead of being distracted by other appointments and patients.

We provide Phone Consultations for clients outside of our house-call area. We can often help you find what is missing by thoroughly reviewing and discussing your pet’s medical & life history, test results, and prior treatments and then recommending different options via a phone consultation.

Yes! We love the opportunity to see ‘healthy’ pets for whom we can ‘adjust’ minor unnoticed imbalances before those imbalances become a significant problem! These apparently healthy pets often benefit from nutrition & lifestyle review & education, acupuncture, and chiropractic care.

No. Instead, we take a holistic-minded approach to health & wellness & prevention.

We use a thorough questionnaire, a detailed discussion with you, and a comprehensive exam of every patient to determine the underlying core problem. Then we choose a treatment modality based on our findings. Sometimes it’s acupuncture, chiropractic, LASER, and rehab exercises, while sometimes, it’s all about herbs and nutrition. It’s usually some combination of these.
